PRACTICE INFORMATION

Good to know.

Payment & eligibility

Deductibles and copays are due at the time of service, as set out in the registration packet. The practice accepts most major credit cards and checks. Self-pay patients and auto accident related claims are not accepted.

Changing an appointment

Please provide at least 24 hours’ notice to cancel or reschedule. A $25 fee applies to missed appointments or cancellations within 24 hours and is charged to the patient, not the insurer.

Transferring medical records

Use the authorization in the packet and ask your previous provider to fax records to (941) 473-3583. The practice cannot accept medical records on disk.

Medicare claims

The practice files Medicare claims and can file with your secondary insurer unless you ask otherwise. Your deductible and applicable coinsurance remain your responsibility.

Referrals & non-covered services

Arrange any required referral or prior authorization before your appointment. You may be responsible for charges when requirements are not met, your plan is out of network, or a service is not covered.
